Hello and welcome. Thank you for taking part in this 10 minute anonymous questionnaire.
The questionnaire aims to explore the perspectives of multidisciplinary healthcare professionals on decision-making capacity assessments. 
 
Eligibility for completing the questionnaire requires that in your current role you have some involvement in decision-making capacity assessments, for example, identifying a need for a patient to have an assessment, conducting the assessment or reviewing the outcome of the assessment.

The Participant Information Sheet (PIS) which can be accessed here, contains important information for participants, and you are required to read it before proceeding with the questionnaire. 

All of your responses will be anonymous and Qualtrics will not collect your IP address.

This study was approved by the University of Auckland Human Participants Ethics Committee on 30th November 2021 for three years. Reference Number 23678.
